Ready to Start Hospice? | HNW COVID-19 Resources
When you call to make a referral, whether it’s for a loved one or yourself, you will need to provide the information listed above.
Tell the person who answers the phone, “We are ready for hospice care.”
Once our team of experts has gathered the information needed, an informational visit will be scheduled.
During a 30-minute conversation, we will share what our team of experts will provide, what to expect while in hospice care, how hospice is paid for by your insurance, and we will obtain informed consent. After this conversation, you will receive a call to schedule an admission visit.
The purpose of the admission visit is to assess the immediate needs of the patient and loved ones, and to ensure the patient meets the medical criteria for hospice care. These needs may include but are not limited to:
