Five Wishes meets the legal requirements for an advance directive in ​Washington. Just like in ​most other states, you can use Five Wishes in Washington to express how you want to be treated if you are seriously ill and unable to speak for yourself, using a document that is easy to understand. All you need to do is check a box, circle a direction, or write a few sentences.

Once it is signed and witnessed, your Five Wishes is a legal document.


The Physician Orders for Life-Sustaining Treatment (POLST) is a portable medical order form that allows any individual with a serious illness or frailty to summarize their wishes regarding life-sustaining treatment.
