Advance Care Planning – Anacortes

Advance Care Planning ensures that your goals, values and preferences are known, not only at the end of life, but if you become unable to communicate those desires due to illness or injury. Participants will learn about Advanced Directives, Health Care Durable Power of Attorney (HCDPOA) and POLST (Physicians Orders for Life-Sustaining Treatment). All documents are provided to you free of charge.
