Dementia Advance Care Planning

As dementia progresses the goals of medical care change and the advance care directive should reflect that.

Dementia directives give goals for early, mid and late stage disease and are ideally completed before dementia develops.  Even people with mild cognitive impairment can have difficulty imagining themselves in the future.
