DNAR Not Required

Having a POLST and opting for DNAR are NOT prerequisites for hospice admission.

Discussing code status can be difficult in the clinic setting. If your patient is seeking care at home, and has 6 months or less to live, “full code” status should not be a barrier to referral to hospice! We can have that difficult conversation with them over time, in the safety of their home, with their circle of support surrounding them.

“There’s nothing more we can do . . .”  has no place in end of life care.

Use language that offers care that is needed now:

  • Urgent and careful management of symptoms
  • Care that comes to your home
  • Support for your family
  • Help with important end-of-life decisions
