The Portable Orders for Life Sustaining Treatment (POLST) form allows you to decide what life-sustaining treatments you want—or don’t want—in an emergency situation where you cannot speak for yourself. Join an expert from Hospice of the Northwest to gain the peace of mind that comes from knowing your voice will always be heard, no matter the circumstances.
